No. A cuff slides onto the outer rim of the ear and holds by light tension, so you can wear one on an unpierced ear or above your existing piercings. If a cuff ever feels loose, we can adjust the tension so it sits secure without pinching.

How does an ear cuff stay on?

By gentle pressure against the edge of the ear, not by clamping. A well-made cuff grips enough to hold through dinner and a car door, and releases with a light pull. The wider dome and openwork styles spread their weight along the ear, so they feel lighter than they look.
